Eden Brown synergy are currently recruiting for BCC who are looking for Qualified Social Worker (QSW) - Adults - Triage Team
Job Title: Qualified Social Worker (QSW) - Adults - Triage Team
Location: Birmingham City
Pay Rate: Up to 28 per hour (Dependent on experience)
Work Schedule: Monday to Friday - Agile working
Key Responsibilities
Triage Support: Assess and triage cases, identifying immediate needs and coordinating appropriate interventions.
Case Management: Manage a caseload of complex cases, providing comprehensive social work support to adults.
Collaboration: Work closely with multidisciplinary teams, external agencies, and stakeholders to ensure holistic and person-centered care.
Assessment and Planning: Conduct assessments and develop care plans in collaboration with service users, ensuring their involvement in decision-making processes.
Agile Working: Embrace a flexible and agile working approach, balancing remote and office-based work to optimize productivity and work-life balance.
Qualifications And Experience
Qualified Social Worker with relevant professional registration (HCPC).
Previous experience in adult social work, ideally within a triage or similar setting.
Strong assessment and case management skills.
Knowledge of relevant legislation and policies in adult social care.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
